I do not accept insurance at this time. In certain cases I can provide a superbill to you for possible reimbursement through your insurance company or employer, who may reimburse all or a portion of the cost.
I accept Flex Spending Account (FSA) cards, Health Savings Account (HSA) cards and health reimbursement arrangement (HRA) plans, which may cover the cost of services, supplements and/or labs.
When using FSA, HSA, and HRA's to pay for services, it is your responsibility to verify coverage for nutrition counseling with a functional nutritionist, labs or supplements. Your plan may require that you have a Letter of Medical Necessity (LMN) from your doctor.
